  • Note that amusement rentals (bouncy houses and the like) are NOT allowed for Sandburg events.
  • Sandburg PTA Insurance Certificate (expires 10-1-24)
  • Important information about outside vendors (DJs, instructors, etc.)
    • Outside vendors (DJs, instructors, etc.) must provide a Certificate of Insurance. This certificate should be emailed to to Pam Knighton (D205 District Receptionist) with a cc to Anna Wick (Sandburg PTA president) and Kathleen Meade (Sandburg PTA President-Elect).
    • Certificates of Insurance are required for:
      • Any vendor that is going to be onsite during an event (like a DJ)
      • Any vendor that is providing equipment for use during an event (like carnival games).
    • Certificates of Insurance are NOT needed for a vendor who is just providing/delivering food (so if you are getting pizza delivered you don’t need liability insurance from the pizza place).
    • If you are not certain if a Certificate of Insurance is required for the specific vendor you are hiring, please reach out to the PTA President and/or President Elect for guidance.
    • Here is a sample certificate to show what the district is looking for: D205 Sample Certificate of Liability Insurance Form. Please note the Certificate Holder section in the lower left corner must read: ELMHURST COMMUNITY UNIT SCHOOL DISTRICT 205, 162 S YORK ROAD, ELMHURST, IL 60126.
    • Important Notes: 
      • If the Certificate of Insurance is not on file with the district prior to your event, the district will cancel your event. 
      • Bouncy Houses (or the like) are NOT ALLOWED.
